人免疫球蛋白分子由两个相同的重链和相同的轻链组成，重链定义免疫球蛋白类别(IgG、IgA、IgM、IgD和IgE)，轻链共价连接到重链上。在健康个体中，血清中大多数轻链与重链结合。然而，在正常人的血清中发现低水平的游离轻链，因为成熟的B细胞产生的游离轻链多于重链。在血清中，FLCκ主要作为分子量为22.5KDa的单体存在，FLCλ作为分子量为45KDa的二聚体存在。这种大小差异导致肾小球滤过率的差异，因此血清中FLCκ与FLCλ的比率为1∶1.6。肾脏对低分子蛋白质的重吸收强烈影响FLC浓度，因此健康人的尿FLC水平较低。

FLC是B淋巴细胞的天然产物，因此，代表肿瘤和反应性B细胞相关疾病的独特生物标志物。FLCs增加与恶性血浆失调和其他淋巴细胞相关的免疫增生性疾病有关。FLCs的检测对多种单克隆丙种球蛋白病是重要的诊断辅助手段，如多发性骨髓瘤、瓦尔登斯特伦巨球蛋白血症、非分泌性骨髓瘤、多发性骨髓瘤、意义不明的单克隆丙种球蛋白病。血清和/或尿液中单克隆游离轻链的精确测量在轻链疾病中尤其重要，例如轻链骨髓瘤、原发性系统性淀粉样变性和轻链沉积疾病。定量单克隆FLC的能力可能有助于监测疾病。轻链骨髓瘤患者主要产生轻链κ或λ，导致疾病早期FLC κ/λ比值的显著变化。尿液中单克隆κ或λ游离免疫球蛋白轻链的检测，也称为本斯琼斯蛋白(BJP)，对于鉴别和监测B细胞恶性肿瘤也很重要。

此外，与健康状态相比，多克隆FLC的合成在某些炎性或自身免疫疾病(例如系统性红斑狼疮、类风湿性关节炎或多发性硬化症，以及癌症、糖尿病和艾滋病)中发现的与B细胞活化相关的条件下显著增加。

背景说明	Mox1 and the glycoprotein gp91-phox are largely related proteins that are essential components of the NADPH oxidase. The superoxide-generating NADPH oxidase is present in phagocytes, neuroepithelial bodies, vascular smooth muscle cells and endothelial cells. It includes a membrane-bound flavocytochrome containing two subunits, gp91-phox and p22-phox, and the cytosolic proteins p47-phox and p67-phox. During activation of the NADPH oxidase, p47-phox and p67-phox migrate to the plasma membrane, where they associate with the flavocytochrome cytochrome b558 to form the active enzyme complex. The p22- and gp91-phox subunits also function as surface O2 sensors that initiate cellular signaling in response to hypoxic conditions.
