心肌标志物

心肌标志物是心肌损伤后血液中水平升高的物质,检测心肌标志物可以评估心肌损伤的程度。心肌损伤标志物主要有肌钙蛋白T (troponin T)、肌钙蛋白I (troponin I)、脂肪酸结合蛋白(FABP)和肌红蛋白(myoglobin)等。

肌钙蛋白是由肌钙蛋白C、肌钙蛋白I及肌钙蛋白T 3个亚单位组成的复合物。肌钙蛋白I和肌钙蛋白T都有三种亚型:快骨骼肌亚型、慢骨骼肌亚型和心肌亚型,骨骼肌和心肌中的TnC是相同的。从序列看,心肌肌钙蛋白I与骨骼肌亚型完全不同。由于这种组织特异性,心肌肌钙蛋白I可以作为心肌损伤的一种很好的标志物。
